Needed for Participation

• You can register your student for a middle school sport by going to the Genesis Parent Access, "Forms" section (Link Here).

• In order to participate your child must have the following items turned in, in person only, to Mr. Watson (C-100) or the Main Office. Forms should not be emailed. A copy of the forms can be found at this link, in the nurse's office window, or with Mr. Watson (C-100).

1.  Physical Examination Form completed by a NJ Physician, APN, or PA. (good for 365 days if one is already on file at school)
2.   Health History/Health History Update Questionnaire completed/signed by the parent (must be completed before every season

Be advised that all portions of the Physical Examination Form must be completed and documented on the physical paperwork by the physician. That includes but is not limited to; the eye examination, pulse, and physician’s cardiac module date. The student athlete is not approved to participate in athletics until all of the paperwork is submitted and approved by our School Physician. Once approved your Genesis account will then be changed to “approved”.
