Sports Clearance Requirements
The following steps must be completed in order for your child to participate in a sport at Winslow Twp. Middle School. Please read all of the information below. Registration is now open for the following:
Current 6th & 7th Graders: MS Fall Sports (Soccer, Girls Field Hockey, Cross Country, Girls Volleyball)
Current 8th Graders: HS Summer Workouts and HS Fall Sports
***Emailed forms will not be accepted. Physical copies of all documents will be required. Allow 48 hours for forms to be processed***
All necessary forms should be returned to Mr. Watson in H-103

- A parent/guardian must register their child on their Genesis Parent Portal account under the “Forms” section. Once complete, you must turn in a copy of the parent Registration Acknowledgement Form linked here.
*All agreements and signatures must be made by the parent or legal guardian.
Step 2: NJ Physical Clearance (click here)
- The NJ Physical Clearance link above contains the Health History form, the Physical Examination form, and the Preparticipation Physical Evaluation Medical Eligibility Form. These forms must be completed by a parent/guardian AND a physician/healthcare provider.
* Only the completed Preparticipation Physical Evaluation Medical Eligibility Form (with physician signature and stamp) and the Registration Acknowledgement Form from Step 1 are to be returned to Mr. Watson (H103) at the middle school. The physical and health history form should remain at the doctor's office where the exam was completed.
* Athletes who have already have a valid physical on file w/ complete medical eligibility form, will only need to submit the health history form from the packet in this section for their new sport.
Step 3: Additional medical support forms as needed
- Students with varying pre-existing conditions will need additional medical clearance forms completed by a physician. These forms should be returned to the middle school along with the Preparticipation Physical Evaluation Medical Eligibility Form and the Registration Acknowledgement Form.
