Sports Clearance Requirements
The following steps must be completed in order for your child to participate in a sport at Winslow Twp. Middle School. Please read all of the information below. ***ALL Winter Sports Will Begin Monday, November 17th***

***Emailed forms will not be accepted. Physical copies of all documents will be required. Allow 48 hours for forms to be processed***
COHORT A: All necessary forms should be returned to Mr. Watson in C-116
COHORT B: All necessary forms should be returned to Mr. Cox in D-113

- A parent/guardian must fully complete and submit the online registration Google Form. Be sure to complete every section: visiting and reviewing the links, and providing your name where required. Upon completion of the Google Form registration, please print and sign the parent/guardian Registration Acknowledgement Form (click here), and return to Mr. Watson in C116 (Cohort A) or Mr. Cox in D113 (Cohort B).
*All agreements and signatures must be made by the parent or legal guardian.
Step 2: NJ Physical Clearance (click here)
- The NJ Physical Clearance link above contains the Health History form, the Physical Examination form, and the Preparticipation Physical Evaluation Medical Eligibility Form. These forms must be completed by a parent/guardian AND a physician/healthcare provider. The physician completing the physical examination should keep the Health History Form and the Physical Examination Form for their records.
* Only the completed Preparticipation Physical Evaluation Medical Eligibility Form (with physician signature and stamp) and the Registration Acknowledgement Form from Step 1 are to be returned to Mr. Watson C116 or Mr. Cox D113, at the middle school. The physical and health history form should remain at the doctor's office where the exam was completed.
* Athletes who have already have a valid physical on file w/ complete medical eligibility form, will only need to submit the health history form from the packet in this section for their new sport.
Step 3: Additional medical support forms as needed
- Students with varying pre-existing conditions will need additional medical clearance forms completed by a physician. These forms should be returned to the middle school along with the Preparticipation Physical Evaluation Medical Eligibility Form and the Registration Acknowledgement Form.
